The Website is operated by The UroFill® Clinic (“UroFill Clinic,” “we,” “us,” or “our”).

By accessing or using the Website, you agree to these Terms and our Privacy Policy. If you do not agree, do not use the Website.

A physician-patient relationship is established only after UroFill Clinic or an affiliated provider has:

Clinical decisions concerning the amount of product administered during a particular visit remain within the professional judgment of the treating provider.

13. Appointment Cancellation and Rescheduling

Appointments may be subject to cancellation, rescheduling, late-arrival and no-show policies disclosed when the appointment is made.

You are responsible for reviewing those policies before confirming an appointment or making a payment.

UroFill Clinic may reschedule or cancel an appointment because of:

  • Provider availability.

  • Safety concerns.

  • Incomplete intake requirements.

  • Medical ineligibility.

  • Severe weather.

  • Emergencies.

  • Equipment or product availability.

  • Events outside our reasonable control.

Unless expressly agreed in writing, UroFill Clinic is not responsible for airfare, lodging, transportation, lost wages or other travel expenses resulting from a changed or canceled appointment.

Patients traveling from outside the Miami area should avoid making nonrefundable travel arrangements until their appointment and treatment plan have been confirmed.

14. Financing

Financing may be offered through independent third-party lenders or financing companies.

UroFill Clinic does not control and is not responsible for a financing provider’s:

  • Approval decisions.

  • Credit requirements.

  • Interest rates.

  • Fees.

  • Payment schedules.

  • Privacy practices.

  • Collection activities.

  • Terms and conditions.

Financing is subject to the third party’s approval and agreements. A financing application may involve a credit inquiry.

You should review the lender’s terms and privacy policy before applying.

Approval for financing does not establish eligibility for treatment.

15. Communications

After you opt out, we may send one message confirming your request. You may continue to receive communications through other authorized channels.

Your mobile information and SMS consent records will not be sold or shared with unaffiliated third parties for their independent text-message marketing.

Wireless carriers are not responsible for delayed or undelivered messages.
